1.)    This a Christmas tree Making competition and open to all affiliated official MX3 distribution outlets such as pharmacy outlets, Supermarkets and product distributors/ trade partners, in which their business is registered in the Philippines are allowed to join this nationwide competition.

• a.    During registration, only One (1) Authorized/ Official representative per outlet, in which only the authorized representative will be the main recipient for the prizes and other allowed claims, based on either of the selected category, Big or Small Category.
• b.    For group registration, one (1) Authorized/ Official Representative is allowed to register per category.
• c.    Organizations that have sub-offices or branches, One (1) Authorized/ Official representative for every registered account.
• d.   Every registered account must be geographically identified in the joined category. Failure to comply the location will automatically invalidate the registered account.
• e.    All official representatives indicated in the registration must have a valid Philippine Passport (minimum of Six (6) months issued validity), secondary ID such as Driver’s License and PRC ID, and should be at least 21 years old and above.

2.)    Registration will be opened from August 1, 2023 to December 15, 2023 via https://www.mx3ph.com/bmsregistration.

3.)    The main material to be used in creating the lantern are empty MX3 variants boxes such as MX3 Capsule 60s, MX3 Plus 30s, MX3 Tea 20s, MX3 Coffee 10s and MX3 Choco 10s. The creative visuals of the lantern should be at least 60% from the MX3 boxes used.

4.)    For this competition, there are 2 categories to choose from, Big Category (6 Feet) or Small Category (4 Feet). The entire length/ height of the Christmas tree starts from the tree topper (Star) and down to the bottom. Artificial lightings and other decorative materials may be added to enhance visual display/ aesthetic appeal of the Christmas tree.

SUBMISSION OF OFFICIAL ENTRIES

CRITERIA FOR JUDGING

13.)    All cash prizes are VAT exclusive. Any claims by the registered participant should be aware of the following conditions:

• a.   All prize winnings are non-transferrable, and that non-monetary prizes are non-convertible to cash. Only the registered name of official representative submitted during the registration has the sole right to claim the prize won.
• b.    To maintain the security of the claims of the registered participants especially those receiving with Cash, The bank account details must be of the same registered name of the official representative indicated during the registration of entry. Winner’s claims can’t be substituted with another representative, only the name registered can claim or the prize will be automatically forfeited.
• c.    Authorization to another representative will not be honored to claim for winning or for non- winning entries.
• d.    The registered address should be the same mailing/ shipping address, otherwise, any prize claims requesting to be shipped to another address will be forfeited.
• e.    The company will immediately notify the winners via text, e-mail, or voice call. Prizes NOT claimed within Ninety (90) days from date of notification is automatically forfeited.

14.)    All registered participants shall abide by the contest’s mechanics and other implementing rules to be set by DMI Medical Supply Co., Inc. that will be fair and just while this contest runs on the valid contest period.

16.)    The company will be providing the schedule date for travel to Israel anytime. The winner will be notified at the earliest time so to give way for the preparation of the needed travel documents and other travel arrangements necessary for the said trip. In the event that the person can’t join the Israel tour, due to no fault of the company, the Israel tour prize shall be automatically forfeited.

17.)    The Travel is limited to Holy Land Israel only and will not be substituted with another destination. This tour is only available to all First placers from each category and must be Philippine passport holders with not less than Six (6) months of passport validity.

18.)    The company shall pay for airfare accommodation (origin of location-Israel, vice versa) and hotel accommodation while in Israel or during the guided tours in any parts of Israel. The tour participant will receive an allowance from the company.

19.)    This tour is non-transferrable, cannot be represented by another person and is non-convertible to cash or in kind. The right to avail travel to Holy Land Israel is automatically forfeited if the registered name of the participant is not available on the date of travel.

20.)    The company can move the schedule any time or be temporarily put on hold especially on occasions wherein travel to Israel is not permitted (e.g. war, civil unrest, pandemic or other global announcement that prohibits travel outside the country). Travel will resume once it is safe for the participants to join the travel.

21.)    The company is not liable to any damage, injuries/ sickness of the participant while on tour. It is the responsibility of the participant to take care of themselves, to be at the best of their health and has medical clearance for the tour. If on medication or is sensitive to the environment, the participant should be mindful of the do’s and don’ts affecting his/her over-all well-being while being part of the tour.

22.)    Employees of DMI Medical Supply Co., Inc. and members of the immediate family of any of the employees up to the Third degree, are not eligible to participate and win. The term “immediate family” includes spouses, siblings, parents, children, grandparents, and grandchildren, whether as “in-laws,” or by current or past marriage(s), remarriage(s), adoption, co-habitation or other family extension, and any other persons residing at the same household whether or not related to the Company’s employees.
